Pharmacodynamics

Atovaquone acts as a competitive inhibitor of ubiquinol, selectively blocking the mitochondrial electron transport chain at the bc1 complex. This inhibition leads to mitochondrial dysfunction and suppression of purine and pyrimidine biosynthesis, resulting in protozoal death.

Proguanil is a biguanide derivative that is metabolized into cycloguanil, its active form. It inhibits parasitic dihydrofolate reductase, disrupting DNA synthesis. Proguanil provides causal prophylactic, suppressive, and curative activity against protozoal infections.

Directions for Use

  • Tablets may be given whole or split, as required

  • Always administer with a fatty diet to improve Atovaquone absorption

  • When used with Azithromycin:

    • Give Azithromycin on an empty stomach, preferably in the morning

  • Complete the full treatment course to prevent relapse

Side Effects

  • Atovaquone has a favourable safety profile with no commonly reported adverse effects

  • Proguanil also has a high safety margin

Rare effects with prolonged use may include:

  • Temporary hair loss
